New Forms Document Students Leaving Special Ed Services
Dropouts and Parents Revocation of Consent

Two new forms to be used to document cases when students leave Special Education Services are now available on Webkidss. One form is called Revocation of Consent and is to be used when parents choose to withdraw their child from special education. The other form is a letter that should be provided to the parent when a special education student drops out of school. These forms will replace the previously used Removal Letter.
