Electronic Signature Request

Directions for Teachers/Providers/Psychs: 

 

For meetings held virtually, you have the option to obtain electronic signatures from parents/legal guardians and other meeting participants. With this option, they have the ability to sign from a tablet, phone, or computer. If you prefer our office to send it, follow the procedures below:

 

 

Before making a request for forms to be emailed to parents for signatures, please do the following:
            1. Discuss with parents/legal guardians the option to send documents electronically. Explain that they will

            receive an email and they can type their signature from their smart phone, tablet, or computer. They must be

            able to access their email to do this. 

            2. Get permission from parents to send documents electronically. Ideally, this would be done at the same

            time you make contact to set the meeting date.

            3. Get valid email address from parents. Double check the accuracy of the email. 

4. Inform parents of the following: “The email subject line will say "Electronic signature requested" and will

be from one of our electronic signature facilitators. The email should arrive within 1-2 days of the meeting.

If the email does not show up in their inbox, it may be located in the Spam/Junk folder. The parents will

click on the attached link, which will take them through the signing process. 

5. They should only sign electronically if they give consent. If they have questions or do not give consent, they need to reach out to you, the provider, to discuss further.

Please note: ALL PAPERWORK and the IEP must be completely filled out prior to being sent electronically to the parents/legal guardians. The IEP/paperwork should not be sent for signature until AFTER the meeting, allowing the parent and the team to give input and changes made as needed. Parents will review the completed forms and once signed, they are considered complete and the final draft. 

 

What happens next?

-The electronic signature facilitator assigned for your district will send the completed forms to be signed to the parent(s)/legal guardians. 

-The electronic signature facilitator will include your email address which means you will receive all completed, signed documents via email. 

-Once documents are signed, you will need to submit remaining paperwork to the Interlocal office. 

-Please  submit to the Interlocal:

1. Checklist and write on the checklist: Parent signature pages submitted electronically by ___________(electronic facilitator assigned to your district/building)

            2. TIP Page

            3. Any remaining paperwork you may need to turn in to complete the student’s file that you would normally

            turn in. For example this could include, the IEP signature page that includes all of the participants listed,

            Gen ed interventions, etc.

 

You DO NOT have to turn in any paperwork included in the file sent out for electronic signature. Your facilitator will already have a copy and upload the documents to Filebound. Only turn in paperwork that was physically signed and/or additional to the IEP forms already sent out.
